Mr. Alwyn Brown, during a hearing which was held on October 22, 2015, also advised the OCG that

the only document utilized by St. Ann Municipal Corporation to record the contract description, the

contract value, the contract commencement and completion dates and all other pertinent details

concerning the contract, inclusive of the contractor’s signature in respect of the works, is not a contract

document, but what is regarded as an “SR Form”. The following was stated during the course of the

referenced hearing:

One of the problems I am

having is the

reference to these documents as contracts. As I

am aware, we refer to these documents as SR

Forms or payment vouchers, right. The persons

who sign these are not necessarily the persons

who actually carry out the work.

Since I have been working at the St. Ann Parish

Council, as I said before, these documents are

usually referred to SR Form or payment

vouchers. We have actual contract documents

that are used when we are giving out a contract

to somebody, per se, to carry out certain works.

So the term, using the term contracts now is

causing a little difficulty for me. Because it is

leaving me in a state of confusion,
